California’s largest immigration detention center, located in California City northeast of Los Angeles, has failed to comply with a federal judge’s orders to provide adequate healthcare, according to a court-appointed monitor’s report filed on Monday, July 28th, 2026.

The investigation was ordered in February 2026 following a lawsuit alleging medical neglect at the facility. The judge required the government to demonstrate compliance across eight critical healthcare components, including staffing, timely medical screenings, primary care assessments, specialist access, emergency services, continuous care, medication access, and a responsive sick call system.

Dr. Muthusamy Anandkumar, the court-appointed monitor, found the facility out of compliance in all eight areas. He wrote, “The facility lacks a reliable system to consistently provide adequate health care, placing individuals at serious risk of both immediate and long-term harm.”

The report identified major flaws such as significant gaps in emergency care, delays in medication provision, inadequate health condition screenings, and insufficient accommodations for detainees with disabilities.

Tess Borden, managing attorney at the Prison Law Office representing the detained individuals who sued the government, described the findings as a “nightmarish story,” citing psychiatric patients left without follow-up, improperly stored medication and culture specimens, and missed or unscheduled specialist appointments.

Despite these findings, a spokesperson named Gustin stated that the facility follows established clinical protocols for managing medications, with pharmacy staff reviewing orders and working with prescribers to ensure continuity of treatment.

The investigation is notable as the first court-ordered inquiry into the full scope of medical care at any Immigration and Customs Enforcement (ICE) facility in the United States. The report arrives amid record-high immigration arrests under the Trump administration, with over 65,000 people in ICE detention as of July 11th, 2026, raising concerns about overcrowding and dangerous conditions nationwide.
